I tend to agree. In all of the cases I have been involved in over an 18 year period, the situation was pretty cut and dry: Either the practitioner had it or didn't, and the ones who didn't had other issues and multiple issues.

There is one exception involving a substance abuse case where no patients were injured. A suspension was appropriate, at least in my opinion.

However, your first point is the most important. Quality most usually involves systems analysis and repair. Good observation!
